There are two current offers for this card, one is for US airways preferred members and the other is for the general public.

Preferred members only: 30,000 US Airways miles after first use, 10,000 miles on your card anniversary and up to 10,000 miles for balance transfers ($1 transferred = 1 mile within the first 90 days, 0% introductory APR for the first fifteen billing cycles and a transfer fee of $10 or 3%, whichever is greater). Two $99 companion tickets, one free US Airways Club day pass, per year and first class check in and zone 2 boarding on all US Airways flights. Annual fee of $89 not waived.

As you can see, the general public offer does not come with the 10,000 anniversary bonus, otherwise the offers are identical.

You have 60 days to cancel your card after being billed for the annual fee, this gives you enough time to receive the 10,000 anniversary mile bonus and still cancel your card to avoid the $89 annual fee.
