I’m not sure what I’ve done wrong. I have Prime(page linked above says I’m eligible), an item > $5 and small, and chose to pickup at a locker, but still unable to apply promo code to purchase.

To simplify, you need to order something only sold by amazon AND small enough to fit in an amazon locker. If you are a non prime member, you will need to spend at least $25 to get free shipping. You can’t use the $5 off on gift cards but you can add a gift card (a physical one – not sure if a digital one would count) onto your order to get you over the $25 for free shipping.

My non-prime account is eligible but my wife’s prime account is not eligible. Weird.

Not Weird, they don’t want you to buy a $5 item and get it for free. By putting the offer on non prime accounts, you must buy a minimum of $25 to get free shipping or upgrade to prime.

I got the offer. Any idea if there’s items for around $5+ that are free shipping without prime membership?

Minimum for free shipping without prime is $25. Also, you can only buy items that are sold by Amazon and not 3rd party sellers.
